Please fill out this form and fax or bring to the office from which you would like your records released. Dr. Hamilton’s information has been filled out to assist you with this process. All records can be faxed to 763-302-2430.

FMLA forms should be requested from your employer or short term disability company. Once received, you can fax to our office at 763-302-2430 Attn: Sarah L. or be dropped off at our office. Please allow 5-7 business days for forms to be filled out. Please include any pertinent dates (e.g. planned first day off and estimated planned return to work date) when you send forms to our office to assist us in completing these as accurately and in as timely a fashion as possible.

Before your Appointment

If you are a new patient, please arrive to your appointment early in order to complete the registration process. In addition, please bring the following:

1. Any paperwork that was sent to you from our office in completed form
2. Photo ID
3. Insurance Information
4. Referral Information (If required by Insurer)
5. Please bring any copies of Operation Records, Medical Records, Report of X-Ray’s, MRIs and CT
6. A list of current medications and any known allergies
7. Scans from Prior Doctor visits pertinent to why are you coming for evaluation.
8. If possible, please bring any imaging studies on CD that were not performed at a TCO location so Dr. Hamilton can view them in person.

Please refer to our new patient section above. In addition, if you are are being seen as a second opinion, it is especially important to provide us with as much information about care you’ve previously received. Please make sure you fill out the release of medical records form noted above to obtain operative and clinic notes regarding care up to this point. If possible, please bring any imaging studies performed on a disc. This includes X-rays, CTs, MRIs as well as any intra-operative imaging performed. Bringing records with you or making sure they are received by our office prior to your consultation with help us provide you with the most meaningful appointment and prevent any delays in developing a treatment plan moving forward.

After your Appointment

For all refill requests, please contact your pharmacy. If it is for narcotic pain medications, please call Sarah at 763-302-2430. All narcotic prescriptions must be picked up in clinic in person (or by a designated friend or family member) due to current laws. We can not call or fax these prescriptions to your pharmacy. Please make sure you allow 1-2 business days for refills.
