Jake Conrad, PT, DPT grew up in Mount Horeb, WI before attending the University of Wisconsin – Madison for both undergraduate and graduate school. He went on to complete an orthopedic residency program in Madison before moving to Minnesota with his wife and dog, Millie. Jake works closely with his patients to develop individualized treatment plans to meet their unique goals, utilizing functional exercise, patient education, and a variety of hands-on manual techniques. He has experience working with a wide range of orthopedic conditions in patients of all ages and understands the importance of forming a meaningful relationship throughout the rehabilitation process.
Outside the clinic, Jake enjoys attending concerts, cheering on Wisconsin sports teams, traveling, snowboarding, and spending time on the lake with his wife and family.
