TCO offers multiple scheduling options to fit your preferences. From calling to booking online, you can schedule 24/7. Patient Forms

• Prior to your appointment, you will receive patient forms via text and email. Please complete these before your appointment.
• The forms include organization policies and consents, as well as health history questions to help us provide the best possible care.
• If you are unable to complete these forms in advance, please make sure to arrive 20 minutes before your scheduled appointment to complete them in person.

Arriving Early

Please arrive early for your appointment.

• Follow up patients should arrive 10 minutes early.
• Physical therapy patients should arrive 15 minutes early.
• New patients should arrive 20 minutes early.

What to Bring

At check-in, please provide your driver’s license, current insurance card, medication list, and any co-pay if relevant.
