Dr. O’Keefe is a board certified orthopedic surgeon with over 15 years of experience in minimally invasive joint replacement including direct anterior hip replacement, knee and shoulder replacement. His expertise includes standard and reverse shoulder replacement as well as partial knee replacement.
Dr. O’Keefe also specializes in arthroscopic procedures of the shoulder and knee including rotator cuff repair and labral repair as well as ACL reconstruction and meniscus repair. In addition, Dr. O’Keefe practices a breadth of general orthopedics including fracture care, hand surgery and foot and ankle care.
Dr. O’Keefe began his training at Macalester College in St. Paul, Minnesota. He graduated with a degree in Biology. He then attended the University of Minnesota Medical School in Minneapolis, Minnesota. Dr. O’Keefe spent two years of Medical School participating in a Graduate Assistantship in the Department of Orthopedic Surgery.
During Medical School, Dr. O’Keefe earned several honors including a RAVITCH Scholarship from the Minnesota Medical Foundation, a Park Nicollet Clinic Scholarship Award and participated in research earning a Minnesota Medical Foundation research grant resulting in two publications in cancer research. These articles were published in Nature Medicine and the Journal of Orthopaedic Surgery.
Following Medical School, Dr. O’Keefe completed his internship and residency at Loyola University Medical Center in Chicago, Illinois. Here he received comprehensive training in the latest surgical techniques including direct anterior hip replacement and advanced arthroscopic techniques. In addition, Dr. O’Keefe received extensive training in fracture care, hand surgery and general orthopedics. Dr. O’Keefe completed his residency having completed nearly 1400 cases.
With extensive exposure to orthopedic trauma care, Dr. O’Keefe spent time in Ho Chi Minh, Vietnam at The Hospital for Traumatology and Orthopaedics participating in orthopedic trauma cases.
In addition to the comprehensive specialized training Dr. O’Keefe acquired, he utilizes over 15 years of clinical experience to provide the best individualized care to each and every patient. His goal is to maximize function and quality of life for those he treats in his practice.

Dr. O’Keefe has contributed to some excellent publications throughout his career.
This past fall, I ended up tearing my ACL during a football game. In general, you never want to see or experience any kind of injury, and especially so being an athlete. My story starts off on the night of September 8th, 2023. On this night we happened to be running a screenplay. As I was driving my guy down, I planted my foot and attempted to rotate it with my knee and then all of a sudden I felt a POP! In the second quarter of the second game of the season, with 3 minutes and 38 seconds until halftime, I hit the ground and was in immediate pain. When Heidi Beckendorf (our trainer at Anoka), Kate Cayton, and Dr. Patrick O’Keefe got out onto the field to meet me, they asked what was wrong. I responded by telling them that, “I felt a bad pop in my left knee.”
From that night on I can still remember the look on their faces, and I knew that something was seriously wrong and I began to imagine the worst. As soon as my mom saw me hopping over and getting onto the medical cart, she knew that it must have been bad. Right then and there it started to hit me that this is really serious. Seeing the look of concern on everyone’s faces when we started to drive off is still imprinted in my brain.
Once I got back to the locker room, they pulled my mom aside and told her what was wrong. They came in, and my mom was in tears. Seeing my mom cry only made me cry more. They told me that my season was over and that I most likely tore multiple ligaments in my knee. Wow. Just like that, my season was over and it was probably my biggest year for recruitment too.
After I got home from the game all of my emotions came out. I was a mess. I had an appointment that next Tuesday to find out what really happened. I was honestly very relieved to find out that I had torn only my ACL due to the fact that I was originally told that I likely had multiple tears in my knee. To this day, I can still remember the phone call with my head coach after we found out the diagnosis. He said to me, “I’m sorry bud, it will be difficult, but I know that you’ll be able to get through it and come back even better next year.”
My surgery was a success and went very well. When I was about 2-3 weeks Post-OP, one of the physical therapists in the clinic that I had the opportunity to meet and work with thought that I was six weeks into recovery, instead of being only 2-3 weeks. Hearing that was a huge confidence booster for me!
Physical therapy and everything else was going great up until about a month after the surgery. I woke up one Sunday morning and one of my incisions was burning and there appeared to be a giant cyst there. Later that day, the cyst popped open and started secreting lots of pus. My incision stayed like that for a week and kept oozing, but seemed like it was getting better. The doctors said that the issue occurring was due to a stitch that wasn’t dissolving, which kept my incision from closing up and healing. It looked as if it were getting better, but one Wednesday during after-school PT, Kate was taking a look at my incision and things weren’t looking good. The look on everyone’s faces when Kate said that things were taking a turn for the worse had me shell-shocked. I didn’t know what to think.
That next day I had an emergency appointment with Dr. O’Keefe, and he said that I needed to have another surgery ASAP because my incision was infected.
I was making a lot of progress in physical therapy, and it was looking good, but one random night I woke up writhing in pain because my knee had suddenly popped. That morning when I woke up my knee was completely stiff, and I couldn’t walk at all. I was told to get in for an MRI as soon as possible and we found out that I now had a slight tear in my meniscus. I am currently on a meal plan and am taking physical therapy very intently because I have learned that only I can control how fast I am able to return to sports and fulfill my aspirations of playing in college.
One of the biggest accomplishments I have had so far is being able to run after having my injury. The feeling of running after knowing that only four months ago I couldn’t even walk, it’s one of the best feelings that you can get. It has made me realize all of the hard work I have put in to come back is finally starting to pay off.
Going through all of this adversity has been difficult, although in a certain sense, I am grateful for the opportunities and new doors this journey has opened up for me.
I cannot thank Twin Cities Orthopedics enough for everything that they have done for me, although the person from TCO who has had the biggest impact on me is Kate Cayton. I couldn’t have asked for anyone better than her. I always look forward to going to PT, and always being able to have a conversation with her. She has been there for me at my lowest points with recovery and she has always supported me. I don’t think that I could ever repay her for everything that she has done for myself and for me to come back to playing sports.
I would also like to thank Dr. O’Keefe. He showed that TCO really does care about their patients. One thing that really stood out to me was during the time when I was in the hospital, and there was the possibility of me needing a third surgery; he didn’t want anyone but himself to do the surgery, had I ended up needing it. He wants to stay with me until the end of my recovery, and I can’t thank him enough for showing that he does truly care about people that he operates on.

I first came to TCO for a cortisone shot in my right shoulder, due to a bicycle accident which happened years before. I was treated for this at another facility and the shots provided no relief. I have now had two shots at TCO and both gave me relief for almost two years, each time. I started having wrist pain in my left wrist and was diagnosed with inflammed tendons. I was given a shot and told that if it doesn’t work well, I will need surgery. Ultimately surgery had to be done with Dr. O’Keefe. I am one week post-op now. I have no pain and my range of motion is returning.

I have been seeing Dr. O’Keefe for several years. In 2012 he fixed my broken femur, which is doing great. I have always had great confidence in him and when he did my knee, he did not disappoint. Before my surgery, I felt well informed and well prepared. The pre-surgery class at Mercy Hospital was so helpful. After my surgery, I had severe muscle cramps and nausea. Dalton, his PA-C, and the nurses worked very hard at figuring out a combination of meds that finally kept both things at bay. Courtney and Marie in PT have been fantastic. Dr. O’Keefe first operated on me when I broke my elbow in a bicycle accident. He was successful in putting my shattered elbow back together. I also have seen him for a knee problem over the last few years. After injections did not solve the problem, I had a total knee replacement that was very successful. I had my left hip totally replaced on March 20th, 2017. This was my second hip replacement, and I was invited to participate in the EXCEL program at TCO, where I would be having my surgery and recovery done in a non-hospital setting. It was a great way to get this done. My surgery was scheduled early in the morning at the surgery center, and after coming out of the anesthesia, I was brought by van to an assisted living facility. There, I had a room to myself with my care giving staff in the room next to mine. They provided excellent around the clock care including checking on my well being and physical therapy sessions. I was ready to go home by 11:00am the very next day.
The staff members I came into contact with through the entire process were all great! Very professional and caring people that were, in my opinion, some of the best care givers out there. I had Dr. O’Keefe as my surgeon and he did a great job with my hip replacement (he had done my right hip back in 2014 as well).
I’d surely recommend the EXCEL program for anyone qualified to do it. For me, it was a great way to get my hip replacement done. I’m enjoying being able to do the activities I enjoy a lot more comfortably now.

I wanted to commend your providers and their excellent staff at your Elk River location. Michael Mastous and Dr. O’Keefe treated my son Liam after a break to both bones in his arm just below the wrist. Liam is a freshman at Elk River High School, and during practice, he suffered a break during a rebounding drill that we all thought would require surgery. It was about 4:00 pm when the break occurred, and I thought your Elk River location accepted walk-ins. When I arrived, the staff politely explained that this was not a walk-in clinic, but they would ask Mike if he would see Liam anyway. Fortunately, Mike and the staff agreed, knowing treating Liam would require them to stay at the office late. After the confirmation via x-ray that both bones were broken, Mike explained that surgery was 50-50 proposition, and regardless, manipulation would be required to splint the arm. Mike diligently proceeded with his staff to manipulate and mobilize the arm via splint. They all assured Liam (who was feeling more depressed than the pain in his arm) that whether surgery, or, if by chance the manipulation lined the bones up properly and surgery could be avoided, that his arm would be fixed. Upon one-week follow-up with Dr. O’Keefe, he showed us and explained that the manipulation lined up the bones perfectly, but, he didn’t want to remove the splint until more healing took place. Dr. O’Keefe was fairly confident that surgery could be avoided.
Surgery was avoided, and at this point in time, the cast is off and Liam is starting to dribble with his left hand. For a kid that practices no less than 2 hours a day, and loves basketball, the care your team provided was not only exemplary, it assured a young man that this was just a small bump in the road. Please extend my sincere gratitude to your fine staff at Elk River, and know that Twin Cities Orthopedics is my only recommendation for Elk River High School athletes who may suffer these types of injuries. Further, in my duties as a high school basketball, football and lacrosse official, I will communicate the fine care we personally experienced at TCO.

After Dr. O’Keefe performed an MRI on me, he told me that the only help was surgery. Dr. O’Keefe has excellent relations with the patients. In August 2016, I saw him 4 weeks after surgery and he ordered 4 weeks of therapy. I had therapy twice a week and did my therapy on days that I didn’t have therapy scheduled. By November, I was at 100%. I am now in Florida trap shooting and playing golf weekly. Thanks again for a great job.

While taking part in an obstacle course mid-September, I suffered a shoulder injury, specifically a dislocated shoulder. After an immediate trip to the emergency room to have my shoulder placed back into the socket, I had to make an appointment to see an orthopedic physician to explore some options. This is when I met Dr. O’Keefe. I explained my situation to him and he had me schedule an MRI, concerned there may be additional damage to the injury site. He was correct. I had also torn my labrum. Again, it was time to explore some options. Surgery was not on my list of things I wanted to do, but Dr. O’Keefe explained it was the best avenue for me and more importantly, why. In a few minutes, Dr. O’Keefe reminded me of the meaningful things in life, which was significant because he spoke of my family, all while my wife and kids were in the room. I agreed to have surgery, followed by physical therapy. Dr. O’Keefe performed the procedure on me, which was a success. While I was in recovery, Dr. O’Keefe went to fill my wife in on how it had gone and said to call with any questions or concerns. This made me feel like I was the only patient Dr. O’Keefe had since it seemed as though he had committed so much time and effort on me since I had first went to see him. Following surgery, the team of physical therapists at Twin Cities Orthopedics in Otsego treated me with dignity and respect. All in all, this mishap, which was not planned, intentional, or desired, gave me the opportunity to meet some great people who had genuine compassion and sincere interest to get me back to normal. I would profoundly recommend having Dr. O’Keefe and the team(s) at Twin Cities Orthopedics facilitate your orthopedic needs.

My knee replacement has been a journey from knee pain, orthoscopic meniscus repair surgery, cortisone shots and finally full left knee replacement. Dr. O’Keefe and his team have been with me through all steps, answering questions, offering support and finally helping make the decision to replace a very worn out knee joint. The decision was mine made on the facts that the knee joint and pain was affecting the quality of my life. I am 72 years old and have places to go, things to do, a loving, supportive husband, children and grandchildren, with whom I want to share my life, not observing from the sidelines.
I strongly recommend attending the recommended pre-operation seminar that gives you a great overview of the hospital stay and process. Do the exercises daily leading up to the surgery and after! Ask questions.
My surgery was on a Wednesday and I was home, with help from my husband, on Friday. Physical therapy in the hospital, at home, and at the clinic has enabled me to walk without a cane at home, except on stairs, within 3 weeks and slowly ride a stationary bike, only for 5 minutes within 4 weeks after the surgery. All of this success is due to the wonderful, positive, supportive team that works with Dr. O’Keefe and Twin Cities Orthopedics. I totally believe that my successful recovery is due to this team and would highly recommend them to anyone with knee joint pain. I had arthritis in the joint and no meniscus. They dealt with all of this and I could not be happier!

Dr. O’Keefe did an excellent job of repairing my broken elbow that resulted from a bicycle accident. The follow up was appropriate. I completed occupational therapy at Twin Cities Orthopedics with good results.
