Practice Overview

Nishant Dwivedi, MD, is a fellowship-trained orthopedic surgeon who specializes in the surgical and non-surgical care of hand and upper extremity conditions. His clinical practice focuses on cutting-edge treatment of degenerative and traumatic hand, wrist, and elbow conditions including arthritis, fractures, ligament and tendon injuries, Dupuytren’s contracture, joint replacement, microsurgery, and minimally invasive techniques including arthroscopy. Dr. Dwivedi also has a special interest in the management of peripheral nerve compressive neuropathies including carpal tunnel syndrome, cubital tunnel syndrome, and lower extremity nerve compression.

Dr. Dwivedi attended medical school in Boston, MA where he met his wife Mollie, a Minnesota native. Following medical school, he completed his Orthopedic Surgery Residency at the prestigious Washington University in St. Louis. While in surgical residency, Dr. Dwivedi had the opportunity to complete additional training focusing on microsurgery at Ganga Hospital in Coimbatore, India, an international leader in upper extremity trauma care and microsurgery. He then completed a Hand and Microvascular Surgery Fellowship at the University of Washington/Harborview Medical Center, a nationally renowned center for comprehensive care of upper extremity conditions in both adults and children.

Dr. Dwivedi strongly believes in compassionate, patient-centered care as the cornerstone of surgical practice. He strives to truly understand each patient’s concerns and individual goals in order to form a personalized treatment plan to help patients return to optimal health and activity. Maintaining surgical excellence, shared decision-making, and connecting meaningfully with his patients are guiding principles of his practice.

Clinical Interest

Hand care, including fracture care, ligament and tendon injuries, hand arthritis, thumb CMC joint osteoarthritis, mucous and ganglion cysts, ligament and tendon injuries, Dupuytren’s contracture, trigger finger, nerve injuries, and carpal tunnel syndrome

Hand procedures, including open reduction and internal fixation (ORIF) of hand and finger fractures, malunions, and nonunions; closed reduction and percutaneous fixation (CRPF) of hand and finger fractures; ligament repair and reconstruction; Dupuytren’s partial palmar fasciectomy, Xiaflex infection, and needle aponeurotomy; excision of hand masses (eg. mucous and ganglion cysts); joint fusion; small joint replacement; thumb carpometacarpal (CMC) joint arthroplasty using ligament reconstruction and tendon interposition (LRTI), suture suspensionplasty, and mini-tightrope techniques; tendon repair and transfer, A1 pulley release for trigger finger; nerve repair – primary, autograft, and allograft repair. In-office wide-awake, local-anesthesia no-tourniquet (WALANT) procedures including: trigger finger release, mucous or ganglion cyst excision, mass excision, DeQuervain’s release.

Wrist Care, including wrist and forearm fracture care, wrist arthritis, DRUJ arthritis, carpal instability, ligament and tendon injuries, and excision of wrist masses (eg. ganglion cysts)

Wrist procedures, including open reduction and screw fixation (ORIF) of scaphoid fractures, scaphoid nonunion repair with bone grafting, proximal row carpectomy, scaphoid excision and 4-corner fusion, total wrist fusion; complex distal radius and ulna fractures, forearm fractures, and carpal fractures; Scapholunate ligament repair and reconstruction; minimally-invasive wrist arthroscopy; TFCC repair; DRUJ arthritis treatment; wrist mass excision (eg. ganglion cysts); carpal tunnel release – primary and revision, open and endoscopic. In-office wide-awake local-anesthesia, no-tourniquet (WALANT) procedures including: mini-open carpal tunnel release.

Nerve care, including primary and recurrent carpal tunnel syndrome, primary and recurrent cubital tunnel syndrome, proximal median nerve compression, radial tunnel syndrome, hand and wrist nerve injuries, lower extremity nerve compression

Nerve procedures, including carpal tunnel release – primary and revision, open and endoscopic; proximal median nerve decompression; radial tunnel decompression; cubital tunnel decompression – primary and revision, anterior ulnar nerve transposition; nerve repair – primary, autograft, and allograft repair; common peroneal nerve decompression; tarsal tunnel decompression

Elbow care, including management of olecranon fractures, radial head and neck fractures, ligament and tendon injuries, lateral and medial epicondylitis, and cubital tunnel syndrome

Elbow procedures, including open reduction internal fixation (ORIF) of olecranon fractures, radial head and neck fractures; radial head replacement; ligament and tendon repair; lateral epicondylitis debridement; cubital tunnel decompression – primary and revision

Shoulder care, including fracture care and rotator cuff tears

Shoulder procedures, including open reduction internal fixation (ORIF) of clavicle (collarbone) and shoulder fractures, shoulder arthroscopy, subacromial decompression, distal clavicle excision, and rotator cuff repair

Ankle care, including management of ankle fractures

Ankle procedures, including open reduction internal fixation (ORIF) of ankle fractures

I had made an appointment in December 2024 for surgery with a general Orthopedic surgeon for my right thumb joint due to arthritis. He had quickly described the surgery to me, and I asked him if he thought I could have both thumbs done by the end of the year. He said that he thought it should be fine. I had made an appointment with Dr. Dwivedi to talk about surgery for my left thumb joint, since the original surgeon didn’t have any openings.

My first impression of Dr. Dwivedi was that he was warm, caring, and gentle. He took his time explaining the surgery, the Ligament Reconstruction, and the Tendon Interposition (LRTI) in a detailed way that I fully understood what to expect. He highly recommended that I did not get both thumbs done so close together. With his thorough explanation, I understood and agreed. I chose to reschedule my original surgery for my right thumb with Dr. Dwivedi and I couldn’t have been happier. I was nervous the day of the surgery, but he was upbeat, calm, and reassuring. I trusted him completely. The night of the surgery, he even called me to see how I was doing.

I am a person who asks a lot of questions until I fully understand things and he was very patient. He seemed happy and excited to share his knowledge with me. My husband and I could tell that he is passionate with his job and takes pride in his work. Every appointment, I was met with his gentle handshake, warm smile, and friendly demeanor. My sister works at the surgery center where the surgery was performed, and she has heard so many great things about him. My hand therapist also said she has heard the same about Dr. Dwivedi. I am so thankful that I chose to have the surgery performed by Dr. Dwivedi! I would highly recommend him and will have him do my left hand sometime this year.

I had surgery with Dr. Dwivedi to transfer a tendon to my thumb after a tendon ruptured and I couldn’t move my thumb. I had a really great experience with Dr. Dwivedi and the healing went great. Dr. Dwivedi is an excellent surgeon who is caring and thorough during all his follow-up visits. He is a polite and courteous surgeon who cares about his patients. I had a great time with therapy after my surgery, and I also realized that the quality of a surgeon is most important for recovery. I would highly recommend TCO for any orthopedic issues.

Although my index finger had been bent by arthritis for a few years, the pain was new. Pain escalated until the slightest bump felt like shards of glass in my finger. I was referred to Dr. Nishant Dwivedi by Melissa Olson, hand therapist.

At my first appointment, Dr. Dwivedi showed me the x-rays and explained no therapy or splint would help, and that I needed surgery. A few weeks later, Dr. Dwivedi fused a joint in my finger. I have not had even the slightest pain since surgery. My finger is straight and pain free! Melissa taught me several exercises and massage therapies, which kept the incision soft and gave me great range of motion in that finger. It’s amazing how the absence of pain in one finger can make every day so much better!
